Connecticut Integrated Care for Kids

The CT InCK program comes out of a federal grant with the goal of improving the quality of care of children & youth birth-21 and pregnant and postpartum individuals covered by Medicaid.


The InCK Intensive Care Coordinator (ICC) provides intensive care coordination for individuals who meet the above criteria living in specific New Haven zip codes. The ICC works to engage, educate, & empower families to help them address their physical, mental, & social drivers of health needs. The ICC provides wraparound care coordination services and serves as the main point of contact for members and families and their care team.


Beginning in early 2023, referrals for the CT InCK program can be made by community providers or families.CT InCK works with children from birth to age 21 with any chronic condition that is expected to last 12 months or more.
