South Central Medical Home Initiative for Children and Youth with Special Health Care Needs
The South Central Medical Home Initiative for Children and Youth with Special Health Care Needs (SCMHI Program) works with children from birth to age 21 with any chronic condition that is expected to last 12 months or more.
Care Coordinators work with any child or adolescent with a medical, physical, mental health, or behavior health condition. This includes children and young adults with multiple providers where parents and caregivers need help coordinating all of their child's care, newly diagnosed patients who may need support and/or education surrounding their new diagnosis, and families who need help connecting to resources, services, and care related to their child's special healthcare need.
The SCMHI Program helps parents and caregivers:
We believe, as parents or caregivers, that you are the experts in your child’s care and must be involved in the decision-making. The SCMHI Program is a coordinated approach which includes home visits, phone calls, medical office visits and more.
